What Is Randomised Controlled Trial?

A study design in which participants are randomly assigned to either a treatment group or a control (placebo) group, considered the gold standard for evaluating the efficacy of an intervention.

Why It Matters for Supplement Brands

RCTs are the strongest form of evidence for substantiating supplement claims. The FDA, FTC, and regulatory bodies worldwide consider well-designed RCTs the most reliable way to establish cause-and-effect relationships between an ingredient and a health outcome. When building a substantiation dossier, RCTs carry the most weight.

How It Works

In a well-designed supplement RCT:

1. **Randomisation**: Participants are randomly assigned to groups, reducing selection bias. 2. **Control group**: Usually receives a placebo (identical-looking inert substance) for comparison. 3. **Blinding**: Ideally double-blind — neither participants nor researchers know who receives the active ingredient. 4. **Oral supplementation**: For supplement claims, the study should use oral dosing in capsule, tablet, or powder form. 5. **Healthy subjects**: For structure–function claims, studies on healthy (non-diseased) populations are most relevant. 6. **Statistical analysis**: Pre-specified primary and secondary endpoints are analysed for statistical significance.

The hierarchy of evidence quality: double-blind RCT > single-blind RCT > open-label RCT > observational study > animal study > in vitro study.

Common Mistakes to Avoid

  • Citing observational studies as if they prove causation (they show association only)
  • Not checking whether the RCT used a relevant dose, form, and population for your product
  • Ignoring study quality — small sample sizes, poor blinding, and high dropout rates weaken evidence
  • Confusing statistical significance with clinical relevance
  • Not reviewing the full text — abstracts can be misleading about study quality and findings
